This activity involves pointing at target objects by stretching your arm and stating out loud, “Such and such is OK” at important points in the work in order to proceed with work safely and correctly.
long-term–low-level (LTLL) contamination, such as that to which aircrew are exposed, has been shown to be more injurious than short-term acute exposure. Aircrew breathe
the low-level toxicity every minute of their working days. When an aircraft lands the passengers disembark, but the crew turn around and do it all over again, time and time again. Perhaps there has too much emphasis on fume events, while the real damage is being done by LTLL.
I am presenting new facts on the cause of the aircraft stall, leading to the accident.
A few hundred thousand dollar fix for the Challenger space ship deferred cost about a billion dollar loss. Building a generative safety culture is a longterm fix.
Establishing a safety culture in the organization is the most effective means of safety assurance in the long term. Humans play a vital role in the running of the processes and are also the cause of most errors. There are a number of reasons for both errors and violations. Instead of increasing controls in terms of engineering controls and SOP’s, it is beneficial for the organization to implement a generative safety culture, wherein safety becomes a way of life and there is little oversight or control required. Losses are prevented at every stage and hazards are actively identified and risk mitigated.

ET 302 took off from Addis Ababa, the capital of Ethiopia on 10th March 2019 and 06 minutes into the flight met with an accident. The aircraft was the Boeing’s latest addition to the B-737 series, the B-737 8Max. The infamous MCAS(software) was to have played a role in the accident and nations all over […]
Why does fear set in when our plan fails? Had we acted in advance, there wouldn’t have been panic.
In my opinion the primary cause of the ET302 accident lies in the take off roll , the final nose dive could have been caused due to structural failure of the elevator and in an extremely improbable case, the infamous MCAS would have had a role to play. All aircrafts are loaded with passengers, cargo […]
My repeated proposals for UPRT training were dismissed till the accident happened. When operation of flight takes precedence over safety.
