Toxic Cabin air, the cause of neurological problems
long-term–low-level (LTLL) contamination, such as that to which aircrew are exposed, has been shown to be more injurious than short-term acute exposure. Aircrew breathe
the low-level toxicity every minute of their working days. When an aircraft lands the passengers disembark, but the crew turn around and do it all over again, time and time again. Perhaps there has too much emphasis on fume events, while the real damage is being done by LTLL.
Flydubai Boeing 737-800 stopped after attempting to take off at Moscow airport
The flyDubai B-737 attempted takeoff without clearance while a Russian Aeroflot A330 was crossing the runway. Euro Control has introduced Runway Status Lights being used at Paris airport, which is a safety barrier for such incidents.
Erroneous takeoff performance: El Al flight takes off with a 40Tonne error@mindFly
The El Al B-789 managed to get airborne safely despite the 40 Tonne error in weight while calculating the takeoff performance.
I have written a paper on how and why humans commit such grave errors and ways and means to prevent them.
To Triage or Not to Triage post an aircraft accident?
Triage after an accident determines the time, treater & treater requirement of the victim. Is it beneficial to create a triage or simply rush the accident victims to any hospital available?
Aeroflot Superjet. Go-around when against all odds, : mindFly
Crew flying an aircraft with multiple failures would be reluctant to carry out a go-around. The human factor.
Default heuristic can help crew decision favouring a go-around if unstable
At 1000ft where the crew has to take a decision, what if the default mode is to call for a go-around and if parameters meet the safe criteria, the crew continue to land? Will this be a safer option than the current method which is the reverse of this?
Air Canada overflight of the taxiway, near disaster?:mindFly analysis
Inattentional blindness, Cognitive bias and poor crew training standards lead to the near world worst air disaster.
